Help us to help you….

You may have noticed that we now ask for a brief outline of your problem when you call to make an appointment at the practice. This isn’t because our reception staff are nosy but it’s a new approach that we call care navigation.

Through specialist training, our team can now offer more choice on who to see in the practice and help you get to the right health professional fast. Our receptionists never offer clinical advice or triage; this is about offering you the choice to see other specialists in our practice team if they have the expertise to deal with your problem; often quicker and without the need to see the GP each time.

By working this way, it helps us to free up time for GPs to care for our patients with complex or serious health conditions. More importantly though, it means you are seen by the clinician that is best placed to deal with your problem each time you visit us. The choice is up to you.

For Urgent & Same Day Appointments please contact our TRIAGE NURSES on 01924 784100 or book a Triage Nurse Call Back appointment online and someone will call you to discuss your problem.

The practice can be contacted by EMAIL for non-urgent requests; trinity.medicalcentre@nhs.net You can also use SystmOnline to send a message straight to the GP’s

A PHARMACY could also provide help and advice on many minor ailments and medication queries. You can find your nearest pharmacy by looking on the NHS Choices website.

For repeat SICK NOTES or ACUTE MEDICATION REQUESTS please contact the surgery on 01924 784101 or email us and we may be able to help without needing to see a GP.

Our PRACTICE NURSES & HEALTH CARE ASSISTANTS are fully trained in dealing with lots of long term conditions. This may mean a more productive appointment for you. Call us on 01924 784101 to book.

We have ADVANCED NURSE PRACTITIONERS deal with acute problems and are able to prescribe medication as a GP would. Speak to the TRIAGE team on 01924 784100 to book an appointment with an ANP

For any stress, anxiety, or mood problems please contact the practice to book an appointment with the practice PSYCHIATRIC NURSE. Alternatively, you could contact Wakefield Turning Point, Talking Therapies on 01924 234860

You may be able to book at TELEPHONE APPOINTMENT with a GP instead of having to come to surgery. Speak to our reception staff who will advise you if your problem may be one that can be dealt with over the phone.
