How Do I Sign Up?

Submit using the online form here or visit the surgery for a paper form.

If you would like to request access to someone elses medical records, such as a child or family member, you need to visit the surgery to collect a paper copy of the form.

Dependant on the age of the person whos records you are requesting we may need to see some documentation before we can provide this.

If you would like to grant access of your records to someone else such as a family member or carer, please visit the surgery to sign a consent form to give us permission to grant them access to your records.

If you have forgot your account details use the below link for guidance on resetting your details. The surgery aren’t able to provide you with login details or reset your password, this needs to be done by the user through PATCHS.

How Do I Sign Up?

To get started with using the NHS app, you need to create an account through their website or app. You will be required to prove your identity to create an account on the NHS app.

If you want to link your NHS app to the GP Surgery, you may be asked for a Linkage Key, ODS code and account ID, if you contact us through email and ask for third party links for the NHS app, we will be able to provide you with these details.
