Letters of Support and Reports

Please note this list is subject to change. If you have anything else that is not listed below we are happy to review and provide further advise. Our usual turnaround is 14 working days, however this may take longer due to overwhelming pressure on the NHS.

We can offer a free patient summary if required, this will include your recent illness, blood tests, medication and ongoing medical issues.

If you have one of the below documents for us to complete, please bring this into the surgery and it will be allocated to the GP for completion.

Things we do complete

  • Letter of Support – £30*
  • Bus Pass Forms / Travel Forms – £20
  • Fire Arms Report – £100
  • Police Medical Form – £100
  • Holiday Cancellation Form – £80
  • Private Insurance Claim Form – £100
  • Army/Capita Health Report – £50
  • Childminder Form – £100
  • Criminal Injuries Compensation Form – £50
  • Adoption / Fostering Form – £73.86
  • HGV/Taxi Medical – £150
  • * We can offer a patient summary if required as this will cover your recent illness/Blood tests and Medications and also list your known ongoing medical issues. This is free of charge service.


Things we don’t complete

  • Letter of support for PIP / Job Centre – Once you have completed your application and returned this they will write to us directly to request more information if required. They do not accept GP letters towards an application.
  • Proof of ID Letters for bank/home office/driving license
  • Lasting Power of Attorney Letters/Counter Signatures
  • Passport Signatures
  • Jury Service Letters
  • Fit to Fly letters/forms
  • Letters for VISA applications
