We can offer a free patient summary if required, this will include your recent illness, blood tests, medication and ongoing medical issues.

If you have one of the below documents for us to complete, please bring this into the surgery and it will be allocated to the GP for completion.

- * We can offer a patient summary if required as this will cover your recent illness/Blood tests and Medications and also list your known ongoing medical issues. This is free of charge service.
Things we don’t complete
- Letter of support for PIP / Job Centre – Once you have completed your application and returned this they will write to us directly to request more information if required. They do not accept GP letters towards an application.
- Proof of ID Letters for bank/home office/driving license
- Lasting Power of Attorney Letters/Counter Signatures
- Passport Signatures
- Jury Service Letters
- Fit to Fly letters/forms
- Letters for VISA applications
