Subject Access Request – Patients

We are able to offer patients full access to their online records However, we understand that some patients may find it difficult to use online services. If you would like to access your records online, please visit https://www.tmcwakefield.co.uk/online-services/

Timescale

The practice will process your request within 30 days of receiving it, provided that all necessary information has been supplied.

If you request copies of your paper records, the process may take significantly longer. All paper records (Lloyd George’s) are stored off-site with a record storage company, so additional time is needed to retrieve and photocopy them.

Fees

We will not make a charge for the first request for access to your medical records. We may however charge a fee for manifestly unfounded, repeated requests or excessive requests, or if further copies of the information are requested

How will I recieve my data?

We will either send your records through email in a PDF format or we can print them and you will be required to collect them from the surgery.
