Non-urgent advice: Important Notice

This page is only for requesting access to records for children under 16. If you want to request access for a child aged 16 or over, you must visit the surgery in person, and your child will need to provide signed consent for you to access their records.

If you believe a child over 16 does not have the capacity to consent on their own behalf, please arrange an appointment to discuss this with a GP.
