Sicknote Information

If you are experiencing a new health issue and need a sicknote, please self-certify for the first 7 days of your illness. Sicknotes can only be issued if you have been unable to work for more than 7 days. Your employer should not ask for medical evidence that you’ve been ill. Instead they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. This is called self-certification.

You may require an appointment before a sicknote can be issued, which might cause some delays. If there is a delay in issuing your sicknote, it can be backdated to the appropriate start date.

Please provide us with as much information as possible, including the following:
– Who is this sicknote for? (Work or Jobcentre)
– The reason you require a sicknote?
– When this needs to start from?
– How long do you require the sicknote for?
If your previous sicknote has expired and you need an extension, please contact us on PATCHS, email us on [email protected], submit a contact form or call the surgery on 01924 784101 and let us know the duration for which you require the new sicknote.
If you need an amended duties note for your employer, we may be able to issue this without an appointment, but if any appointment is needed we will arrange this. Please contact us on PATCHS, email us on [email protected], submit a contact form or call the surgery on 01924 784101.

-What is the reason you require amended duties
-What amended duties you require
-Timeframe required for the amended duties
We do not issue return to work notes.

You should go back to work as soon as you feel able to and with your employer’s agreement. This may be before the end date on your fit note.

You do not need to see a healthcare professional again to go back to work.

Some employers have their own policy that requires employees to obtain medical evidence that they are fit for work. If this is the case, your employer should help you arrange this privately with a healthcare professional or occupational health specialist. A healthcare professional cannot issue a fit note for this purpose.
