Information for New Patients

Thank you for registering with us – we’re delighted to welcome you to our practice.

Our surgery operates across two sites: Trinity Medical Centre and Sandal Castle Medical Centre.

If you have a preference for which site you’d like to attend for future appointments, just let us know — we’ll do our best to accommodate your choice.

If you’d like a general check-up as a new patient, you’re welcome to request a New Patient Health Check.
You can do this by calling us on 01924 784101, emailing trinity.medicalcentre@nhs.net or sign up to PATCHS to request this.

If you’re currently taking repeat medication from your previous GP, our pharmacy team may need to review it before we can continue prescribing.

To discuss your medication, please email our prescriptions team directly at wyicb-wak.prescriptions.trinity@nhs.net.
If you have any evidence of your previous prescriptions — such as a labelled medication box or a prescription slip — please include this, as it can help speed up the process.

Once your repeat medications have been set up on our system, you’ll be able to request them in a variety of ways. Full details are available on our prescriptions page here.

Please note we are unable to take prescription requests over the phone.
We offer several convenient ways to access our services online, visit our online services page for more detailed information.
So we can update your records, please provide any vaccination history, doctor/hospital letters or any information you feel will better help us care for you. Please note we can not update your covid vaccination status. You can send us this information by visiting our surgery or emailing it to trinity.medicalcentre@nhs.net.
